The East of England renal network are keen to improve the information given to patients who have been admitted to hospital with an acute kidney injury- this is when there is a sudden and recent reduction in kidney function. This may be due to dehydration, infection or blockage to the kidney (amongst other reasons). The network would like to find out more about what information patients are currently given when being discharged from hospital, and what type of information would be useful to have.

We would be grateful if you could fill out the short survey below (the survey should take less than 5 minutes to complete). This will help us design discharge information that would best serve patient needs and help answer the most frequently asked questions.
