If you are a renal patient (CKD, pre-dialysis, Dialysis or Transplanted) or a carer of a renal patient, then the NKF would like you to complete this survey – anonymously.
When the results have been analysed the NKF will pass the results to the NKF Executive Committee for their information and possible action.
Please phone the NKF Helpline (number at the foot of the page) should this survey give rise to any concern.
This survey is confidential – you do not have to identify yourself.

The NKF is a Federation made up of about 70 smaller Kidney Patient Charities, many of whom seek money from the public. The NKF does not currently seek financial support from the public.
